Position Overview:

We’re seeking a Senior PHP Developer who brings both deep technical expertise and a forward-thinking “build with AI” mindset. We need a strategist who understands how to integrate AI tools across the entire development lifecycle to accelerate delivery, improve code quality, and solve complex architectural challenges.
You’ll be taking ownership of ongoing complex projects, architecting secure solutions for sensitive medical data and setting technical standards for our development practices. This role requires someone who can hit the ground running with minimal handholding and make critical decisions in a regulated, high-stakes environment.

Key Responsibilities:

• Take full ownership of complex, ongoing PHP projects for global clients and architect highly secure, enterprise-grade systems handling sensitive healthcare data
• Build scalable solutions from core PHP without over-reliance on frameworks, lead code reviews, and establish coding standards across the team
• Strategically integrate AI tools into development workflows to accelerate delivery, including AI-assisted testing, code refactoring, documentation generation, and automated vulnerability scanning
• Leverage LLMs for intelligent problem-solving and build AI-powered features within applications (intelligent alerting, anomaly detection, predictive analytics)
• Ensure HIPAA, GDPR, and SOC 2 compliance across all codebases, implement robust security measures (encryption, secure authentication, RBAC), and maintain comprehensive audit trails
• Conduct security audits, vulnerability assessments, and penetration testing while handling sensitive PHI/PII data with strict security protocols
• Design high-availability systems with 99.9%+ uptime, implement disaster recovery and business continuity plans, and build multi-tenant architectures with data isolation
• Optimize application performance, database queries, system resource utilization, and implement microservices architectures for distributed healthcare systems
• Inherit and analyze complex, undocumented codebases, reverse-engineer business logic, and develop migration strategies for legacy systems without disrupting production
• Integrate with third-party medical systems (EHR/EMR, HL7/FHIR APIs, medical devices) and evaluate emerging AI development tools to enhance team productivity
• Train team members on effective AI tool usage, prompt engineering techniques, and mentor junior developers through code reviews and knowledge sharing

Qualifications:

• 8+ years of hands-on PHP development with expert-level knowledge of core PHP and ability to build from scratch without framework dependency
• Deep proficiency in PHP 8.x features (attributes, JIT compiler, enums, union types, fibers) and strong command of modern frameworks (Laravel, Symfony) with framework-agnostic approach
• Advanced OOP principles and design patterns (SOLID, Factory, Repository, Observer, Strategy), plus PHP performance optimization, memory management, and profiling
• Proven experience integrating AI tools into development workflows beyond code completion – hands-on with AI APIs (OpenAI, Anthropic Claude, Google AntiGravity or similar)
• AI-assisted development expertise: code generation, refactoring, testing, debugging, and prompt engineering skills for technical problem-solving
• Experience building AI-powered application features with critical thinking about when AI adds value vs. when manual expertise is essential
• Data encryption, anonymization, and audit logging experience in regulated environments with strict change management
• Understanding of GDPR, SOC 2, ISO 27001 standards and compliance requirements for global firms
• Advanced MySQL/PostgreSQL skills (query optimization, indexing, partitioning), database encryption, secure data handling, and data migration expertise
• Redis for caching, message queues (RabbitMQ/Kafka), multi-tenant database architecture, and schema versioning
• Secure coding practices and OWASP Top 10 vulnerability mitigation with API security expertise (OAuth 2.0, JWT, rate limiting, API key management)
• Experience with SAST/DAST tools (SonarQube, Snyk, Checkmarx), penetration testing, security audit participation, and secure key management (HashiCorp Vault, AWS KMS)
• Microservices architecture, event-driven systems, distributed systems design, and high-availability system implementation
• Legacy system takeover experience: ability to reverse-engineer undocumented codebases, develop refactoring strategies, and manage technical debt
